Requesting Your Medical Records
You have the right to view and/or request copies of your medical record. Copies are provided only after the patient is discharged. Please allow 30 days from the date of discharge for record processing before requesting a copy.
There is no charge for requesting a copy of your medical records for personal use or for medical provider use.
Request your medical records through UCHealth My Health Connection or read below for more options. The quickest and best way to receive your medical records is through My Health Connection.
Mobile App
– Log in to your My Health Connection account on your mobile app.
– Click the three white bars on the top right.
– Select “Tools,” which will drop down options.
– Select “Share my record.”
– Select who you would like to send the copy to: “Yourself” (or “A family member, close friend, or caretaker”).
– Select “Request a formal copy” (or “Download or send a snapshot”).
– Answer the question prompts.
– Click “Continue.”
– You will receive a message when your medical records are ready to be picked up!
Website
– Log in to your My Health Connection account online.
– Select “Your Menu” at the top left of the screen.
– Under “Sharing,” select “Share my record.”
– Select who you would like to send the copy to: “Yourself” (or “A family member, close friend, or caretaker”).
– If you would like to request someone else’s medical record, select “Click here to request access to someone else’s record (for instance, your child).” at the top of the screen underneath “Share my record.”
– Select “Request a formal copy” (or “Download or send a snapshot”).
– Answer the question prompts.
– Click “Continue.”

If you would like a copy of your medical record, you can also download and print a medical request form. To receive copies of your medical record, please submit a completed request form. This form must be signed and may be submitted by fax, mail, e-mail, or in person. Verification of identity is required. Acceptable forms of identification include driver’s license, employment ID, state issued ID, current school ID, military ID, Veteran Affairs ID, or a current passport.
To submit in person:
Drop off the request form at Admissions from 8:00 a.m. – 5:00 p.m. Monday through Friday at Sidney Regional Medical Center’s main campus, located at 1000 Pole Creek Crossing, Sidney, NE 69162. You can also drop off the form at SRMC Patient Financial Services, located at 645 Osage Street, Sidney, NE 69162, from 8:00 a.m. – 5:00 p.m. Monday through Thursday or 8:00 a.m. – 4:00 p.m. on Fridays. Please arrive at least 15 minutes before closing.
