Our team consists of physicians, physician assistants, nurse practitioners, and support staff who work together to certify your health is our number one priority. Telehealth

Telehealth provides patients the option to contact their SRMC provider from the comfort of their homes. Under the guidance of the Centers for Medicare and Medicaid Services, telehealth improves provider access through devices such as a smartphone, tablet, or computer. At the time of your appointment, a provider will connect with you, and the visit will take place through the use of a device and a virtual platform. The Chronic Care Management (CCM) program is a care coordination service that is completed outside of your regular visits with your healthcare provider. This program provides you with around-the-clock access to care that will help you stay on track with your treatments and overall care plan. The CCM program is offered to Medicare patients at little to no cost. Through this program, you will receive the following:

  • An interdisciplinary team of professionals, including the provider, nurses, outpatient services, social worker, and case management who address the physical and social needs by coordinating care.
  • Nurses make routine calls to patients and their families to see if any new symptoms or needs arise and relay this information to their primary provider.
  • If symptoms or pain are unmanageable at home, they may be admitted as an inpatient to the hospital, placed on observation status, placed in a swing bed, or admitted to a nursing home. Further guidance from the Physicians Clinic, Walk-In Clinic, or ER are needed.

Eligibility

If you live with two or more chronic conditions, CCM can help you manage your care. Some examples of chronic conditions include but are not limited to the following:

  • Alzheimer’s Disease
  • Arthritis
  • Cancer
  • COPD
  • Dementia
  • Diabetes
  • Heart Failure
  • Osteoporosis

Additional support provided includes the following:

  • 24/7 support
  • Patient education
  • Transportation for SRMC established patients to attend clinic and outpatient service visits at SRMC

Medicare may cover transitional care management services during the 30-day period that begins when a physician discharges a Medicare patient from an inpatient stay and continues for the next 29 days. This service helps eligible patients transition back to a community setting after a stay at our hospital.

This program is a non-face-to-face service that is done by our CCM staff. The patient is contacted within two business days after the patient is discharged from the inpatient or partial hospitalization setting. Our job is to support patients as they transition to a community setting. We assist with scheduling follow-up appointments from the hospital stay and check on patient status after the hospitalization during this period. Additionally, we educate patients about their treatment, assess patients, support treatment adherence including medication management, and help patients access needed care and services. Please note, TCM services does transition into CCM services after the 30 days have elapsed when needed.

The pharmacy department at SRMC is proud to offer anticoagulation management services with a referral from an SRMC primary care provider. This is a unique service in the panhandle that is oftentimes only found in large university hospitals. In essence, if you are referred by an SRMC primary care provider to the collaborative practice clinic, your medication will be managed by one of SRMC’s highly trained pharmacy staff. You will meet with an SRMC pharmacist no less than once a month where they will provide medication monitoring, expert guidance, answer questions, help create a follow-up schedule, and even send refill information to your pharmacy. A Medicare Annual Wellness Visit (MAWV) is a preventative visit with your provider and a great way to focus on issues or concerns that may be overlooked in a routine physical exam. Together with the Accountable Care Organization (ACO) team, you will develop a long-term preventive care plan to guide your health.

What is a Medicare Annual Wellness Visit?

This is a yearly appointment where the ACO team and provider will update your health records and ensure you are up-to-date on receiving preventive healthcare services like screenings and vaccinations.

Who is eligible?

All Medicare Part B members who have not had a Medicare Annual Wellness Visit or “Welcome to Medicare” visit in the past 12 months.

Is the Medicare Annual Wellness Visit the same thing as a yearly physical?

No, the MAWV is a yearly visit with your provider to create a personalized preventive health plan. However, your provider may elect to perform additional services during a MAWV which could incur additional costs.

What should I do if I haven't had an MAWV in the past 12 months?

Call the SRMC Physicians Clinic at 308.254.5544 and ask to schedule your MAWV. Talk to your provider or nurse about what you can do to prepare for the visit and what they would like you to bring.

We are the first contact of care and coordinate management of health problems requiring multiple professional services.

The Vaccines for Children (VFC) is a federally funded and state-operated vaccine supply program. VFC provides free Advisory Committee on Immunization Practices (ACIP)-recommended vaccines to eligible children. Through the combined efforts of Public Immunization Clinics and Private Physicians, this program strives to increase vaccine coverage levels nationwide.

Who is eligible for the Vaccines for Children program?

Children – Birth through 18 years of age and one falls under one of the following:

  • American Indian or Alaska Native
  • Medicaid enrolled
  • Uninsured (have no health insurance)
  • Underinsured –
    • Children who have commercial (private) health insurance, but the coverage does not include vaccines.
    • Children whose insurance covers only selected vaccines (VFC eligible for non-covered vaccines only).
    • Children whose insurance caps vaccine coverage to a certain amount (once that coverage amount is reached, these children are then categorized as underinsured).
    • Children whose insurance has a very high deductible (catastrophic insurance) are NOT considered underinsured.
  • Underinsured children are eligible to receive VFC vaccine only through a Federally Qualified Health Center (FQHC), Rural Health Center (RHC), or a deputized public immunization clinic.

Providers participating in the VFC program must comply with program requirements on vaccine storage and handling, record-keeping, and reporting to the Nebraska Immunization Program.

As a participating member of the VFC program, we are allowed to charge a vaccine administration fee of $19.82 for each vaccine given to the child. We administer these vaccines to children that are established in our RHC and meet one of the aforementioned criteria.
